Description
O-Desmethyltramadol (commonly referred to as O-DSMT) is the primary active metabolite of tramadol, a synthetic opioid analgesic. It is responsible for much of tramadol’s opioid activity due to its stronger affinity for the μ-opioid receptor. O-DSMT has not been approved as a standalone medication and is subject to varying legal classifications depending on jurisdiction.
In addition to its opioid receptor activity, O-DSMT is associated with effects on neurotransmitter systems involved in pain modulation. As with other opioids, its pharmacological profile carries significant risks, including respiratory depression, sedation, tolerance, physical dependence, and withdrawal. Adverse effects may be intensified when combined with other central nervous system depressants.
